July 2 - Senator Arlen Specter (D-Pa.) urged the Secretary of Veterans Affairs Eric Shinseki to adopt several policies so that patients and the hospital are notified in the event a medical procedure is not successful: "At a minimum, the rules should be explicit on the obligation of the performing doctor to report such mishaps to his supervisors and in turn those mishaps should be reported to the patients..."
